Preservatives in Skincare: The UK Hub

Preservatives keep skincare safe, but they are also the single biggest cause of allergic contact dermatitis in UK dermatology clinics. The plain-English hub - what preservatives do, which ones sensitise most, and how to read a label.

Short answer. Preservatives stop skincare going off, and without them water-based products would grow bacteria and mould within days. But some preservatives are among the most common causes of allergic contact dermatitis in the UK. The three highest-risk categories are the isothiazolinones (MI, MCI), the formaldehyde-releasers, and (much less commonly) certain older systems. If you have reactive skin, these are the names to know.

Why preservatives exist at all

Every water-containing skincare product is a growth medium. Bacteria, mould and yeast will colonise a cream or lotion within days if nothing stops them. Contaminated products can cause skin infections, eye infections, and in vulnerable patients (babies, immunocompromised, broken skin) systemic infections. Skincare that is not preserved has to be single-use, refrigerated with short expiry, or genuinely self-preserving through very low water content (anhydrous balms, powders, oils). Everything else needs a preservative system to be safe on the shelf and safe in your bathroom cabinet.

This is worth understanding because it changes what a “preservative-free” claim means. Sometimes it means the product is a genuine self-preserving anhydrous formulation - which is fine. Sometimes it means the product uses ingredients that preserve without being labelled as “preservatives” (grapefruit seed extract, essential oils, alcohol at high concentration) - which may or may not actually preserve the product safely. Sometimes it means the product is not properly preserved - which is not fine.

Preservatives are a feature of the product, not a flaw. The question is which preservatives, not whether to have them.

The three highest-risk categories

1. Isothiazolinones - MI, MCI

The classic preservative-allergy story of the last twenty years. Methylisothiazolinone (MI) and its chemical cousin methylchloroisothiazolinone (MCI) rose to be the most-used preservatives in personal-care products in the late 2000s and early 2010s. Then the UK/EU allergic-contact-dermatitis rate climbed sharply - patch test clinics across Europe reported the fastest-rising positive rate they had ever seen, driven almost entirely by MI. In 2015 the EU (adopted by the UK) banned MI and MCI in leave-on cosmetics; MI remains legal in rinse-off at up to 15 ppm.

INCI names to know: Methylisothiazolinone, Methylchloroisothiazolinone, Benzisothiazolinone, Octylisothiazolinone. The last two are less common in cosmetics but appear in household products.

Once sensitised to one isothiazolinone, cross-reactions to the others are common, which means avoidance means all of them.

2. Formaldehyde-releasers

A family of preservatives that work by slowly releasing tiny amounts of formaldehyde over the shelf life of the product. Formaldehyde itself is a known contact allergen (and a category 1 IARC carcinogen at higher concentrations), and although the amount released from these preservatives is small, sensitised individuals react to trace exposures. Formaldehyde allergy is one of the more common allergens on UK patch testing.

You may also see plain Formaldehyde in some hair-straightening products (rare and heavily restricted in the UK).

Where they appear: shampoos, conditioners, moisturisers, sunscreens, foundations, nail products. Formaldehyde-releasers are especially common in high-water-content products where they work well.

3. IPBC and paraben-family issues (much less common)

No preservative is universally non-sensitising, but these categories dominate what dermatology-focused UK brands use, and for most reactive-skin buyers they are the ones to look for.

Reading a preservative name on an INCI list

Preservatives sit near the middle or end of the ingredients list. Middle position is unusual - if you see a preservative name early in the list (in the top five), the concentration is probably higher than usual and worth noting for reactive skin.

The two preservative-shopping mistakes worth avoiding

Mistake 1 - going preservative-free for its own sake. Preservatives are what make water-based skincare safe. A “preservative-free” moisturiser bought from a market stall or a small brand can be an unsafe product. Ask what preserves it - if the answer is “essential oils” or “grapefruit seed extract”, those do not consistently preserve at safe levels. If the answer is “we use a proprietary organic-acid system”, ask which acids, then verify on the INCI.

Mistake 2 - equating “paraben-free” with “safe”. Paraben-free products often replace parabens with methylisothiazolinone or a formaldehyde-releaser, both of which sensitise at higher rates than parabens ever did. Paraben-free is a marketing category, not a safety category. Read the INCI to see what is actually in the product.

Common questions

Do I really need preservatives in skincare?

Yes, in almost all cases. Any water-containing product (cleansers, lotions, creams, gels, most sunscreens) is a growth medium for bacteria, mould and yeast within days without a preservative. Contamination can cause skin, eye, and systemic infections - especially in already-broken or eczematous skin. 'Preservative-free' water-based products are usually one of: single-use, refrigerated with short expiry, self-preserving through a very low water activity, or unsafe. Look at how the product is packaged and sold before assuming preservative-free equals safer.

Are parabens really the worst?

No. Parabens have a long safety record and are among the lower-sensitising cosmetic preservatives. The 'parabens are bad' narrative that took hold in the 2010s was based on limited early research on hormonal effects that has not held up, and pushed manufacturers to replace parabens with other preservatives - some of which cause much more allergic contact dermatitis. The unintended consequence of the paraben panic is that the skin-allergy rate for the replacement preservatives has climbed. Parabens are still safe to use for most people; other preservatives are more likely to cause the actual skin reaction.

Is methylisothiazolinone (MI) banned?

Not fully. MI has been banned in leave-on cosmetics in the UK/EU since 2015 following a widespread epidemic of contact dermatitis in the 2010s. It remains legal in rinse-off cosmetics at up to 15 ppm (parts per million) - shampoos, shower gels, cleansers. It also appears in some household products (dishwashing liquids, cleaning products) and industrial products (paints, coolants). MI is still one of the top allergens in UK dermatology patch testing today, driven by the rinse-off exposures.

Which preservative is safest for reactive skin?

Phenoxyethanol has one of the better safety profiles - low sensitisation rates, well-tolerated on most reactive skin. Ethylhexylglycerin is often paired with it and also has a low reaction rate. Certain organic acid systems (sodium levulinate, sodium anisate, glyceryl caprylate) are gentle. No preservative is universally safe - the least bad choice depends on your specific sensitivities.
